The purpose of this Privacy Policy is to describe how iOptimal Functional Medicine collects, uses and shares information about you through our website located at www.ioptimalfm.ca (the “Site”). It explains how we use and disclose that information, the choices you have regarding such use and disclosure, and how you may correct that information.
iOptimal Functional Medicine takes your data privacy seriously, through comprehensive security infrastructure and stringent data policies. WHAT IS PERSONAL INFORMATION?

We collect this information you provide to us, for example when you create or modify your account, register to use our Site, purchase products or services from us, request information from us, contact customer support, or otherwise communicate with us. Please note if you sign up to use our service you will also be asked to submit your Photo ID, as well as demographic and other information relevant to diagnosis and treatment, lifestyle and general medical history.

We will always collect your Personal Information by fair and lawful means. We may collect Personal Information from you directly, where we have obtained your consent, or as otherwise required or permitted by law.

If you are under the age of 18, please do not attempt to register with us at this Site or provide any personal information about yourself to us. We will keep the Personal Information that we collect at our service provider’s data centers within Canada.HOW DO WE USE YOUR PERSONAL INFORMATION?

The intent to collect Personal Information is identified at the time of collection from you, and we obtain your consent, prior to such use. Personal Information is generally collected and used for the following purposes:
to facilitate communications with Nurse Practitioners, and Pharmacists;
provide information that you request from us;
contact you about your appointment/ consultation;
process credit card and debit card transactions;
send you promotional materials or advertisements about our services, as well as new features and offerings;
enforce our Terms of Use or other legal rights and remedies;
administer surveys and promotions;
to investigate legal claims;
provide interest-based targeted advertising to you; and, any other purposes disclosed to you at the time we collect your information or pursuant to your consent, as may be permitted or required by applicable law.
TO WHOM DO WE SHARE YOUR PERSONAL INFORMATION?
Generally, we will only make disclosures of Personal Information to :
Healthcare Providers.
We may transfer your Personal Information to Nurse Practitioners and relevant medical staff in connection with your diagnosis and treatment.
Advertising.
We may use how you browse in order to show you ads for iOptimal Functional Medicine or our advertising partners that are more relevant to your interests. We may use cookies and other information to provide relevant interest-based advertising to you. Interest-based ads are ads presented to you based on your browsing behaviour in order to provide you with ads more tailored to your interests. These interest-based ads may be presented to you while you are browsing our site or third-party sites not owned by iOptimal Functional Medicine.
Social Media.
If you use third-party services, such as Facebook, Google, Instagram, or YouTube, through the Site, or to share information about your experience on the Site with others, these third-party services may be able to collect information about you, including information about your activity on the Site, and they may notify your connections on the third-party services about your use of the Site, in accordance with their own privacy policies.
Public Posts and Activities.
If you choose to engage in public activities on the Site, you should be aware that any information you share there can be read, collected, or used by other users of these areas. We may provide your Personal Information to service providers that: help us operate and manage our Site, Nurse Practitioners and relevant medical staff in connection with your diagnosis and treatment.
